Need to Submit a Claim?

Submitting claims electronically is the easiest and most convenient option. You’ll find steps for submitting Life and Disability, Dental and Vision claims online below. If you prefer to use a manual form, visit our dedicated page to download the appropriate forms and find step-by-step instructions to guide you through the process.

Dental offices should use Electronic Payer ID RLHA1 to submit claims. 

NOTE: If the member number on the ID card is a six-digit number starting with one, call 800-894-4532 for eligibility and customer support.

Solstice DHMO dental claims

Dental offices should use Electronic Payer ID 76578 to submit claims, or email contact@solsticebenefits.com.

VSP doctors will submit claims on the patient’s behalf. To submit an out-of-network claim, visit VSP or call 800-877-7195.
